What is needed to file a restraining order?

My ex-fiancé and I had recently broken-up. We had been staying at my parent’s house waiting for our condo to be built. Long story short we had many arguments within that time frame which lead to us calling off the wedding. When things got scary was when he attempted to force his way through my locked bathroom door while I was in the shower “to talk”. My parents were in the house, witnessed this, stopped him and he left. I called the police to file an incident report, the officer spoke with him on the phone and he agreed to move out.
Since then however, I have been noticing that many of my online (personal) accounts have been hacked and possessions from my car are constantly going missing. Are these incidents, coupled with the report I had filed with the police, enough to get a restraining order against him?
